Pool Social Work Case Manager, Cornell Institute, Bethesda East, Per Diem

Intervenes with patients and families for psychosocial needs, providing consultation, discharge planning, education, counseling, advocacy, and referrals to address emotional, social, and financial aspects of care. Offers crisis intervention, inpatient emotional support, bereavement services, and addresses child or elder abuse and domestic violence. Supports neonatal, infant, pediatric, adolescent, and adult patient groups. Estimated pay $37.25 per hour, shift-dependent.

• Master’s degree (MSW preferred). Social Work case managers hired before 10/1/2017 with a Bachelor’s degree in Social Work are not required to hold an MSW to continue in a non‑leadership role.
• 0–1 year of social work experience in a hospital or related health care setting; emergency department experience preferred. Internship experience considered.
• Ability to solve problems in a fast‑paced environment and work independently with sound judgment.
• Excellent interpersonal, communication, negotiation, analytical, data‑management, computer, and organizational skills.
• Current working knowledge of discharge planning, crisis intervention, complex case management, performance improvement, and community services for insured and non‑insured patients.
• Strong time‑management skills and capacity to prioritize multiple high‑risk, complex patients.
• Ability to collaborate with multiple care team members and maintain positive working relationships.

Minimum required experience:
Less than 1 year.
