Position: Cancer Support Navigator

Job Summary

The Cancer Support Navigator provides direct non-medical care navigation for participants, maintains a robust resource and referral source, manages the Travel Assistance Program, and is actively involved in CSCMT’s relationship with the hospital relationships and other referral sources. The Cancer Support Navigator ensures that policies, procedures, and programs are carried out in a professional manner in keeping with the Cancer Support Community HQ program guidelines.

The Cancer Support Navigator works collaboratively with the entire team.

Essential

Job Duties and Responsibilities Direct Navigation
  • Identify potential and realized barriers to cancer care (e.g., transportation, childcare, eldercare, housing, language, culture, literacy, psychosocial, employment, financial) and facilitate referrals as appropriate to mitigate barriers.
  • Work with Program Coordinators to be the first line of contact with new participants by providing information via phone or in person and provide navigation to help participants gain full access to CSCMT programs.
  • Provide patients and caregivers evidence-based information to support understanding and decision-making along the care continuum.
  • Address and research patient’s needs to provide local resources to enhance patient outcomes and quality of life.
  • Develop, implement, and evaluate the CSCMT Travel Assistance Program in coordination with the entire program/outreach staff.
  • Work with Clinical Director of Programs to inform on current cancer trends and observations.
  • Create and implement a process to ensure detailed documentation of participant interactions and needs.
  • Work with outside partners to create strong referral relationships and smooth referral processes.
  • Collaborate with educational institutions, area hospitals, Chapter colleagues, and the Cancer Support Community National staff in research and development of new clinical programs.
  • Be a participatory member of the Professional Advisory Board.
  • Establish and maintain proactive relationships, and be actively involved, with other CSCHQ mental health professionals through program conference calls, Affiliate Meetings, and other collaboration opportunities.
  • Help oversee the order of the building which includes maintaining an inventory of supplies, serve as point person for maintenance, bulk mailing, shipping, and equipment. Maintain building to be tour-ready at all times in collaboration with team at chapter location.
  • Inventory program materials and resources information, and reorder when necessary. This includes Comfort Totes, library resources, and Frankly Speaking items.
Clinical Social Work (when applicable)
  • Provides immediate emotional support to participants who walk-in and telephone, in distress.
  • Provides virtual/digital programming in addition to or as a replacement for in-person programming, and as the situation dictates.
  • Absorbs and disseminates detailed information and relates it to patients, caregivers, and family members in terms that are understandable in a pleasant and helpful manner.
  • Performs therapy under the supervision of Clinical Director of Programs.
Team Development
  • Actively involved with staff meetings and training sessions and adhere to the Team Values and Commitments.
  • Commitment to working as a positive and supportive team member in a therapeutic community.
  • Collaborate with staff, volunteers and participants to create and maintain a culture of support and empathy while embracing the mission of CSCMT to provide in-house psychosocial education, as well as compliance and training of confidentiality policies and practices.
  • Mentor Mental Health Interns in coordination with Clinical Director of Programs, when appropriate.
  • Work with Clinical Director of Program to educate staff by training, sharing knowledge and adherence to prevailing governmental laws and regulations regarding the provision of mental health services and Ethical and Professional Standards as designed across the mental health disciplines in Montana, including, but not limited to:
    • Confidentiality policy and standards
    • Reporting of child and elder abuse/neglect
    • Maintenance of records
