ResponsibilitiesThe Case Management Nurse Director reports directly to the Chief Nursing Officer and will work closely with physicians, nursing leaders, social services, and interdisciplinary teams throughout the hospital. As a newly opened leadership position, the Case Management Nurse Director will have the opportunity to help guide care coordination efforts while building strong relationships across multiple departments.
The Case Management Nurse Director will oversee hospital case management services focused on safe patient transitions, discharge planning, utilization management, and continuity of care. This role partners with experienced clinicians across the organization to ensure patients receive coordinated, high-quality care from admission through discharge.
The Case Management Nurse Director will provide clinical leadership by mentoring staff, supporting professional development, and promoting evidence-based practices that enhance quality and patient satisfaction. The role also includes responsibility for departmental operations, staffing, performance improvement initiatives, regulatory compliance, physician collaboration, and developing efficient workflows that support both patients and caregivers. Strong leadership and communication skills will be essential in driving positive change throughout the department.
