RN - Case Manager

Description:

The Registered Nurse Non-Certified Case Manager works with physicians and multidisciplinary team members to develop a plan of care for assigned patients.

The role ensures that patients are progressing towards desired outcomes by monitoring care through assessments and/or patient records.

It involves identifying and resolving barriers that hinder effective patient care.

The position requires active involvement in the discharge planning process.

Key responsibilities include completing comprehensive assessments of members per regulated timelines.

The nurse determines who may qualify for case management based on clinical judgment and changes in the member's health or psychosocial wellness.

They will collaboratively develop and implement a case management plan with the member, caregiver, physician, and other healthcare professionals.

Ongoing monitoring of the care plan is essential to evaluate effectiveness, document interventions, and suggest changes as necessary.

The role also promotes integration of services for members, including behavioral healthcare and long-term services.

Candidates should demonstrate sound judgment, patience, and maintain professionalism at all times.

The ability to work in a busy and stressful environment is essential.

Strong interpersonal, verbal, and written communication skills are necessary for this position.

Applicants must have the ability to analyze, organize, and prioritize work while meeting multiple deadlines.

This role requires regular and sustained attendance, with potential for working beyond a standard 40-hour workweek.

Physical requirements include the ability to lift up to 50 pounds, maintain a standing posture for extended periods, and possess good balance and coordination.

Candidates should be comfortable with fine-finger movements and have close visual acuity for preparing and analyzing data.

Exposure to blood borne pathogens and infectious diseases is expected in this position.

The role may involve emotionally challenging situations, such as dealing with distressed patients or difficult family interactions.
