Overview

As a Home Health LPN, your voice to influence patient care is valued and empowered at every turn – through open, collaborative relationships with your direct manager or more formal opportunities through hospital councils and national nursing initiatives. You’ll help shape decisions that elevate both patient outcomes and the future of nursing.

Responsibilities
  • Providing nursing care in the home, including wound care, medication administration, and support during procedures
  • Documenting assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
  • Monitoring patient status and alerting the care team of any changes to ensure timely, coordinated responses
  • Educating patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
  • Collaborating closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting
Qualifications
  • Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
  • Minimum of two years’ experience in an office setting; one year of home health agency experience is preferred
  • Must have ability to obtain licensure in additional states
  • Proficiency with computer use and software applications including MS Office; EMR experience preferred
  • Current BCLS prior to delivering patient care
  • Reliable transportation and proof of valid automobile liability insurance
  • Must have valid driver’s license
