LPN - Field ALF​/LTC North

Full time 40 hours a week.

Coverage area:
Manatee County and North Sarasota County.

Under physician’s orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) observes patient/family needs, provides supportive care, and offers emotional support during crises to maximize comfort and enhance quality of life.

Primary responsibilities:

• Under the direction of the RN case manager, provide continuous bedside clinical observations, prescribed care, and supportive care to patients as directed in the plan of care, reporting significant changes in patient condition and family needs as needed.
• Collaborate with the RN to accept and process physician orders to maximize patient comfort.
• Utilize monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.

Requirements:

• Graduate of an approved nursing program and possess a current Florida LPN license.
• Minimum one year of experience in an acute care setting, providing direct patient care; previous hospice LPN experience preferred.
