• Home visits in assigned service area:
  Manatee County

• Oversee the activities of the multi‑disciplinary team that provides care to the patient
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Ensure that all visits scheduled and provided are authorized and covered by physician orders
• Review patient visits weekly and provide a plan to the Patient Care Supervisor and team assistant, including anticipated weekend visits
• Inform the scheduler of any schedule changes and communicate caseload and patient care issues to the Home Care Supervisor

• BLS (Basic Life Support)
• RN (Registered Nurse) License
• Driver's License of the State of Florida (required within 30 days of hire)
• Associate’s or Diploma in Nursing (required)
• At least 1 year of Registered Nurse experience (required)
• Bachelor’s in Nursing (preferred)
• At least 1 year in Home Care (preferred)
