• Home visits in assigned service area:
  Dunedin, Palm Harbor, Tarpon Springs, Oldsmar, and surrounding areas

• Provide hands‑on care in the home for a group of patients using the nursing process and clinical standards.
• Oversee the activities of the multi‑disciplinary team that provides care to the patient.
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
• Ensure that all visits are authorized and covered by physician orders.
• Review patient visits weekly and provide plans to Patient Care Supervisor and team assistant, including anticipated weekend visits.
• Inform the scheduler of any schedule changes and communicate caseload and patient care issues to Home Care Supervisor.

• Certifications and Licenses: BLS (Basic Life Support), RN license, Driver's License of Florida (within 30 days of hire).
• Education:
  
  Required Associate's or Diploma in Nursing;
  Preferred Bachelors in Nursing.
• Experience:
  
  Required 1 year as Registered Nurse;
  Preferred 1 year in Home Care.

This position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA).
