As a Licensed Practical Nurse (LPN), you will provide direct patient care to the residents and patients of our health and rehabilitation center, assisting them in activities of daily living under the direction and supervision of a professional nurse. Your responsibilities may include:

• Assisting in the implementation of individualized treatment plans for each assigned resident
• Supervising Certified Nursing Assistants (CNAs) and participating in their evaluation
• Overseeing Falls Program and Pressure Sore Program
• Providing nursing services
• Conducting and documenting thorough evaluations of each resident's medical status
• Other duties as assigned

• Must possess a current, unencumbered, active state license to practice as an LPN
