Memory Care Manager (LPN or RN)

Elan Assisted Living is seeking a dedicated and experienced Memory Care Manager (LPN or RN) to lead and oversee our specialized memory care community. This is an excellent opportunity for a compassionate healthcare professional to make a meaningful difference in the lives of residents with memory impairments while working within a supportive and growth-oriented environment.

• Develop, implement, and oversee individualized care plans for residents with memory impairments
• Supervise and coordinate daily activities of the memory care team, ensuring high-quality resident care
• Monitor residents’ health status and respond promptly to changes or concerns
• Ensure compliance with all state and federal regulations, as well as facility policies and procedures
• Provide training and support to staff on best practices in memory care and resident safety
• Foster a warm, respectful, and engaging environment for residents and their families
• Collaborate with healthcare providers, families, and team members to optimize resident well-being

• Valid LPN or RN license in good standing
• Proven experience in memory care, assisted living, or a related healthcare setting
• Strong understanding of dementia and other memory-related conditions
• Excellent leadership, communication, and interpersonal skills
• Ability to assess resident needs and adapt care strategies accordingly
• Knowledge of regulatory requirements and compliance standards
• Compassionate, patient, and dedicated to providing exceptional resident care
