• Provide direct patient care, including assessment, planning, implementation, and evaluation of individualized care plans
• Collaborate with the interdisciplinary team to develop and implement care plans, ensuring continuity of care and optimal patient outcomes
• Perform routine vital signs, administer medications, and provide wound care as needed
• Educate patients and families on disease management, self‑care, and preventive measures
• Document patient information and maintain accurate records
