Registered Nurse; RN) Diabetic Educator

Brief Description

The Registered Nurse (RN) Diabetic Educator is responsible for providing comprehensive diabetes self-management education and support to patients diagnosed with diabetes, pre-diabetes, and other related metabolic conditions. The RN Diabetic Educator collaborates with providers, care teams, and community resources to improve patient outcomes, promote healthy lifestyle changes, and support disease prevention and management.

The RN Diabetic Educator collaborates with providers, care teams, and community resources to improve patient outcomes, promote healthy lifestyle changes, and support disease prevention and management.

REQUIREMENTS & PREFERENCES:

• Minimum of two (2) years of clinical nursing experience required.
• Experience in diabetes management, endocrinology, primary care, community health, or chronic disease management preferred.
• Bachelor of Science in Nursing (BSN) preferred. Associate Degree in Nursing (AON) required.
• Current Registered Nurse (RN) license in the applicable state.
• Current Basic Life Support (BLS) certification required.
• Minimum of two (2) years of clinical nursing experience required.
• Experience in diabetes management, endocrinology, primary care, community health, or chronic disease management preferred. Certified Diabetes Care and Education Specialist (CDCES) preferred or required within a specified period of employment.

The Staff Nurse (RN) ensures services are administered utilizing professional standards with a strong focus on quality, timely care, and exceptional customer service under the direction of the medical provider.
