The Opportunity

We are currently seeking a dedicated Social Services Assistant

What You Will Do

• Assist in the planning, organization, implementation, and evaluation of the facility’s Social Services Department under the direction of a Licensed Social Worker

• Ensure social services are delivered in compliance with federal, state, and local guidelines and in alignment with Harborview's values and policies

• Help develop and monitor individualized care plans for residents, addressing their social and emotional needs

• Provide ongoing support to residents and families during admissions, transitions, and care conferences

• Recommend appropriate equipment, services, or referrals for outpatient and home health support

• Organize and participate in-service training sessions for the social services team and other staff

• Advocate for residents and serve as a liaison among staff, families, and external agencies

• Collaborate with interdisciplinary care teams to ensure holistic, person-centered care

• Strong interpersonal, communication, and problem-solving skills

• Ability to manage sensitive situations with professionalism and compassion
