Universal Primary Care, a Federally Qualified Health Center dedicated to expanding access to high‑quality care, is seeking a Family Medicine Physician to join our outpatient practice in Bradford, PA. This role is ideal for a physician who values community‑based care, team collaboration, and serving patients of all ages in a supportive environment.
Key Responsibilities- Provide comprehensive primary care to patients across the lifespan
- 100% outpatient care if not providing OB services
- Maintain good quality medical records
- Deliver patient‑centered, evidence‑based care in a collaborative, team‑oriented setting
- Maintain accurate and timely documentation within the electronic health record
- Participate in quality improvement initiatives and population‑health efforts
- Diagnose, treat, and monitor diseases, disorders, and conditions
- Prescribe appropriate medications to treat patients in accordance with standardized procedures and company policy
- Perform pap smears, pelvic, and breast exams
- Conduct mood and emotional well‑being assessments during post‑partum appointment
- Support UPC’s mission of accessible, equitable care for all community members
- All other duties as assigned
