Overview
Be part of a dynamic team to provide strength-based community case management services to individuals who possess a mental health diagnosis. Duties include development of recovery-oriented treatment plans, collaboration with assigned treatment team, linking with community-based resources and services, providing advocacy and support for individual recovery journeys, and coordinating with crisis services through an on-call rotation.

- PTO (over 6 weeks per year)
- Two (2) Floating Wellness Holidays
