Job Description
Coordinates the total Plan of Care for patients in the Home Care setting, assessing, planning, implementing, and evaluating. Provides leadership for the Health Care team by working cooperatively with other team members to maintain standards of professional nursing practice.

- Associate’s Degree required.
- Current nursing license required.
- 1–5 years of relevant experience.
