Occupational Therapist - Home Care

Part-Time/On-Call hours. Monday-Friday, NO nights, weekends, or holidays! Paid mileage reimbursement.

• Direct patient care: continuous assessment of patient needs and functional deficits, patient care/treatment, on-going modifications to existing plan of care as needed, ability to initiate appropriate referrals to other disciplines when needed
• Work closely with skilled and dedicated interdisciplinary team in order to maximize patient satisfaction and overall success based on diagnosis and comorbidities
• Apply clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge and communicating with Physicians to ensure fluency of care
• Complete documentation which follows regulatory guidelines (state, federal)

• Graduate of an accredited Occupational Therapy program
• Currently licensed to practice in State of MN. New graduates and temporary license holders are encouraged to apply
• Able to use theoretical knowledge in the rehabilitation of residents through therapeutic sessions.
• Ongoing attendance of educational seminars to maintain license(s) as required.
• Must be patient and appreciative of realistic needs and goals of all clients.
• Genuine interest in persons with physical and psychological disabilities.
• Possess excellent interpersonal, organizational and time management skills.
• Must be able to communicate effectively with members of the health care team, clients, residents, families, visitors, vendors and all others that SLP would have interactions with.
• Must be able to follow policies and procedures as instructed and applicable to job responsibilities
• Must maintain work and client/resident confidentiality.

• Competitive Pay! $39-$50.00/hr + mileage reimbursement. Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related reasons.
