Job Requirements- Education Requirements: Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license from an accredited program
- Certifications Requirements: Valid state-specific LPN/LVN license; CPR certification;
Negative COVID-19 test or vaccination as per facility policy - Experience Requirements: Minimum of 1 year of recent Long-Term Care (LTC) experience;
Prior experience working with elderly populations in a clinical setting;
Familiarity with EMR systems - Other Requirements: Ability to work 3-12 hour shifts;
Strong communication and teamwork skills;
Flexibility to float to different units as needed

- Provide comprehensive nursing care to residents in a Long-Term Care facility, ensuring adherence to care plans and safety standards.
- Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized patient care plans.
- Maintain accurate documentation within the electronic medical record (EMR) system, recording patient assessments, treatments, and progress.
- Administer medications and treatments as prescribed, monitoring for adverse reactions and effectiveness.
- Educate residents and their families on health management, medication compliance, and wellness strategies.
