Job Overview

Position: MDS Nurse – Good Samaritan, Woodland (Brainerd, MN). Shift: 8‑hour day shifts, 32 hours weekly. Salary: $40.00–$51.00 per hour with a $10,000 sign‑on bonus. Union status:
No. The role requires 3 shifts per week as MDS Nurse and 1 shift as RN Staff Nurse, plus RN on‑call rotation.

• Plan, organize, direct, and evaluate activities of the nursing staff involved in resident care plans.
• Evaluate care delivered to each resident and keep care plans current.
• Lead the Resident Assessment Instrument (RAI) process.
• Assist in assessment and evaluation of potential admissions.
• Complete resident assessments quarterly and annually.
• Collaborate with residents, families, and interdisciplinary colleagues to ensure ongoing high‑quality care.
• Use the nursing process (assessment, diagnosis, planning, implementation, evaluation) to provide individualized care.
• Coordinate resident assessment, education, and other nursing interventions.
• Serve as charge nurse when required, delegating tasks to licensed and unlicensed personnel within job descriptions and competence levels.
• Provide assistance and support to ensure safe, reliable resident care and resolve care or service concerns.
• Operate within the scope of nursing practice as outlined by the Nurse Practice Act and state Administrative Rules.

• Graduate from a National League of Nursing (NLN) or American Association of Colleges of Nursing (AACN) accredited program (preferred);
  Bachelor’s degree in nursing preferred.
• Minimum one‑year experience as a licensed nurse with senior population (preferred).
• Prior experience as an MDS Nurse preferred.
• Hold an unencumbered RN license issued by the State Board of Nursing in Minnesota (or multistate licensure if in a Nurse Licensure Compact state).
• Maintain required certifications, continuous education units, and competency training.
• Resident Assessment Coordinator‑Certified (RAC‑CT), management or supervisory experience preferred.
• Affiliation with an outside professional nursing organization is preferred.

• Salary: $40.00–$51.00 per hour.
• Sign‑on bonus: $10,000 (if eligibility requirements are met).
