Licensed Practical Nurse

Essentia Health is seeking a Licensed Practical Nurse to deliver compassionate, patient-centered care in a collaborative clinical team. The LPN will provide direct nursing care including vital signs, medication administration, wound care, and treatment support under RN/physician direction. Responsibilities include patient intake and assessments, documentation in the EMR, assisting with procedures, and reinforcing patient education and discharge instructions. The LPN supports quality, safety, and infection-control standards while promoting holistic care.

Responsibilities

• Provide direct nursing care including vital signs, medications, and treatments under RN/physician direction
• Conduct patient intake, basic assessments, and accurately document in the EMRAssist with clinical procedures and coordinate patient flow in the unit or clinic
• Reinforce patient and family education on treatments, medications, and discharge instructions
• Follow infection-control, safety, and quality standards to ensure a safe care environment
• Communicate changes in patient condition promptly to the RN or provider
• Collaborate with interdisciplinary team members to support holistic, patient-centered care
• Maintain accurate, timely documentation and comply with regulatory and organizational policies
• Support onboarding of new staff by modeling best practices and team collaboration
• Participate in ongoing training and quality improvement initiatives
