MDS Coordinator

Affinity Healthcare is currently hiring for the position of MDS Coordinator.

Job Summary:

the MDS coordinator is responsible for assuring the completion of each resident's MDS and care plan. This includes ongoing assessment of each resident's health care needs and direct communication with care staff to provide and evaluate the effectiveness of the plan of care.

Reports to:

Director of Nursing

40 hour per week, hourly position

• Tracks and schedules required resident assessments per state and federal requirements
• Completes all Minimum Data Sets (MDS) assessments and Care Area Assessment Summaries (CAAs)
• Transmits all completed MDS assessments to the state in compliance with state and federal regulations and Affinity Healthcare policy
• Completes resident assessments to capture highest possible Patient Driven Payment Model (PDPM) score
• Responsible for monthly Case Mix reports and Quality Indicator/Quality Measure Reports
• Monitors documentation in PPC to ensure consistency and compliance with state and federal and AFFINITY Healthcare requirements
• Reviews daily 24 hour reports for significant changes in resident conditions that may require assessments and revised/additional care plans
• Participates in IDT/Risk meetings
• Oversees and communicates resident assessment scheduling changes and status updates
• Oversees the discharge and re-entry process as it relates to MDS
• Reviews and provides feedback regarding care plans (both chronic and acute)
• Ability to work as a nurse, rounding with clinicians, collaborate with the Staff Development Coordinator and educate direct care staff to documentation, assessments
• Other duties as assigned

• Must be a nurse with a license in good standing in the state of Massachusetts
• Prior experience working in a skilled nursing facility
• Prior experience as a MDS coordinator preferred
• RAC-CT preferred
