The Registered Nurse in Home Care at Mass General Brigham provides skilled nursing care in patients' homes, focusing on individualized care plans and collaboration with interdisciplinary teams. This role is ideal for RNs who value autonomy and meaningful patient relationships.

• Pay: $40.09 - $71.12/Hourly
• Location: Braintree, MA
• Facility: Mass General Brigham Home Care, Inc.
• Schedule: Part-time (20 hours), Every Saturday & Sunday, 8:00 AM – 6:30 PM
• Sign-On Bonus: $25,000 weekend bonus

• Lead and coordinate individualized patient care plans
• Deliver 1:1 skilled nursing care in the home
• Collaborate with interdisciplinary clinicians
• Provide advanced clinical treatments and patient education
• Help prevent hospital readmissions and improve long-term outcomes
• Complete starts of care, resumptions of care, evaluations, or revisits
• Perform admissions process and case management responsibilities
• Utilize thorough and timely electronic documentation and interdisciplinary communications
• Provide quality clinical services to patients in various community settings

• Education: Graduate of an approved School of Nursing
• Experience: Minimum of one year previous nursing experience in an acute care setting;
  Prior home care experience preferred
