Home Health Clinical Manager

As our Home Health Clinical Supervisor, you'll guide and support field clinicians, coordinate high-quality patient care, and help your team deliver the outcomes patients deserve all while staying connected to the mission that brought you into nursing.
Clinical Magnet, a division of ICON Medical Network, is looking for a compassionate, dedicated Home Health Clinical Manager to join our partner's full-time team in Braintree, Massachusetts.

- Overseeing patient care, clinical staffing, referrals, and assignments
- Ensuring timely starts of care and seamless care coordination
- Reviewing plans of care, physician orders, and clinical documentation
- Conducting OASIS reviews to support accuracy, quality, and compliance
- Monitoring patient needs and ensuring care plans remain appropriate
- Coaching clinicians and providing ongoing education and feedback
- Supporting recruiting, interviewing, onboarding, and staff development
- Partnering with physicians, patients, families, providers, and vendors
- Monitoring key clinical and operational performance metrics
- Providing field support when needed
- Unrestricted Massachusetts RN License
- 2+ years of RN experience in Home Health or Hospice
- Supervisory or clinical leadership experience preferred
- Strong OASIS, care coordination, and clinical knowledge
- Ability to learn systems such as Homecare Homebase and Workday
- Willingness to travel within branch's coverage area
- $95,000 - $120,000 annually
