Continuum Hospice is seeking a self‑motivated and passionate Hospice RN Clinical Liaison to join our Team. The RN Clinical Liaison is primarily responsible for hospital and medical facility‑based outreach to identify new referral sources and maintain ongoing relationships that will increase census and preference for hospice services.
