Care Partners, a leading home health care provider in Ontario, seeks Team Coordinators for immediate full-time and part-time roles. You will coordinate schedules for Personal Support Workers, Nurses, and Therapists while providing exceptional customer service. Your focus on timely administrative support will ensure efficient service delivery to our patients.
Key Responsibilities:
• Act as the communication bridge between staff and families
• Receive and process intake information from clients
• Develop and coordinate service schedules effectively
• Enter relevant data into the automated system
• Manage inbound communication regarding service inquiries
Requirements:
• One year of administrative or customer service experience
• Post-secondary education preferred
• Proficient in Microsoft Office, especially Excel
• Exceptional interpersonal skills and attention to detail
• Ability to work non-traditional hours, including weekends
