Florida Kidney Physicians (FKP) is seeking a
Dialysis Outreach Coordinator to serve as the primary liaison between our vascular access surgery center and dialysis clinics throughout the Greater Tampa area. This unique hybrid role combines relationship management and community outreach with front office coordination, making it ideal for someone with dialysis experience who is passionate about improving patient outcomes.
Approximately 75% of your time will be spent building strong relationships with dialysis clinics, educating clinical teams on vascular access services, coordinating timely patient referrals, and ensuring patients receive prompt intervention when access issues arise. The remaining 25% will support patient scheduling and front office operations at our Brandon Surgery Center.

What You’ll Do
Outreach & Relationship Management (Approximately 75%)
- Develop and maintain strong relationships with dialysis clinics, nephrology offices, and healthcare partners throughout the Greater Tampa market.
- Conduct regular in-person visits to dialysis clinics to educate staff on FKP's vascular access services.
- Serve as a trusted clinical resource regarding vascular access preservation and timely intervention.
- Promote awareness of early identification and treatment of failing AV fistulas and grafts.
- Collaborate with dialysis teams to facilitate timely referrals for patients experiencing vascular access complications.
- Build referral partnerships through exceptional customer service and consistent communication.
- Track outreach activities and referral trends while identifying opportunities for growth.
- Represent FKP professionally within the nephrology and dialysis community.
Access Coordination & Front Office Support (Approximately 25%)
- Coordinate patient referrals and scheduling for vascular access procedures.
- Serve as a primary point of contact for patients, dialysis clinics, and physician offices.
- Verify referral information and ensure all required documentation is obtained.
- Assist with patient registration, scheduling, insurance verification, and appointment coordination.
- Maintain accurate patient records and facilitate communication between providers and patients.
- Support front desk operations to ensure an exceptional patient experience.
Qualifications
- High school diploma or equivalent required.
- Associate's degree or healthcare-related education preferred.
- Minimum of 2 years of dialysis experience required.
- Clinical dialysis experience strongly preferred (Dialysis Technician, Patient Care Technician, Medical Assistant, Licensed Practical Nurse, or similar).
- Previous experience working with vascular access patients preferred.
- Experience building professional relationships with healthcare providers or clinics is highly preferred.
- Strong understanding of dialysis access, including AV fistulas, AV grafts, and dialysis catheters.
- Excellent interpersonal, communication, and customer service skills.
- Self-motivated with the ability to work independently while managing a territory.
- Comfortable traveling throughout the Greater Tampa area several days per week.
- Valid driver's license and reliable transportation required.
- Proficiency with Microsoft Office and electronic medical records preferred.
