Position Summary

$5000 Sign-on Commitment Bonus
- West Pasco - Reporting to the Clinical Care Manager, the Hospice RN
- Field is responsible for providing quality care to patients with Gulfside Healthcare Services, in the field. The field includes patients who reside in private residences, assisted living facilities, and skilled nursing homes. The Hospice RN utilizes assessment, intervention, and education to maximize patient comfort and to enhance the quality of life for the patient/family. The Hospice RN
- Field oversees and case manages the needs of the patients assigned to them, provides supervision for the L.P.N.’s, C.N.A.’s, and other assigned staff, maintains clinical education requirements, meets professional standards, and acts as an advocate for patients/families.

Education and Qualifications
  • Currently licensed in the State of Florida as a registered nurse
  • Minimum of one (1) year nursing experience in a hospital, home health or hospice setting preferred
  • Strong ethical standards and appropriate professional demeanor
  • Sound critical thinking, time management, technical skills, analytical ability, good judgment, and strong organization
  • Team‑oriented, well‑organized, and self‑directed
  • Reliable transportation with proof of automobile insurance and a valid Florida driver’s license

Job Responsibilities
  • Case‑manage the needs of patients and families in the field
  • Perform ongoing assessment of physical, psychological, social, and spiritual needs; initiate appropriate intervention
  • Provide or supervise nursing and activity staff (P.N.’s, C.N.A.’s, etc.)
  • Identify problems and take independent action to resolve them
  • Support policies, procedures, and orientation of new staff
  • Serve as a resource person, mentor, and guide for co‑workers and additional staff
  • Maintain positive relationships with interdisciplinary team members
  • Respond to patient/family complaints and concerns
  • Develop initiatives to strengthen relationships with facility partners
  • Educate the community about hospice care, philosophy, and concepts through individual consultations
  • Communicate regularly with the Clinical Care Manager and assess risk management areas
  • Maintain current organizational communication via e‑mail and check accounts at the start of each shift
  • Collaborate and coordinate with colleagues and management regarding nursing process and service issues
  • Create and implement nursing care plans, review daily, and update as needed
  • Consider financial implications to provide cost‑effective nursing care
  • Ensure professional nursing practice standards are maintained
  • Complete required documentation before ending the scheduled shift
  • Participate in IDG (Interdisciplinary Group) and communicate patient needs appropriately to other disciplines
  • Collaborate daily with the team physician on patient status
  • Perform other duties as requested by the Nurse Care Manager
Skills and Competencies
  • Maintain up‑to‑date skill sets required for patient care
  • Multi‑task, adapt, and demonstrate flexibility to meet Gulfside Healthcare Services objectives
  • Ensure compliance with patient care and documentation standards and regulations
  • Assist in implementing and maintaining clinical department policies and procedures, and in evaluating them
  • Assist in maintaining federal and state regulatory compliance
  • Proficiency in Electronic Medical Records
Licenses and Certifications
  • CPR
  • RN
