POSITION SUMMARY:

Reporting to the Clinical Care Manager, Crisis Care LPN is committed to providing the highest quality of care as identified in the Gulfside Healthcare Services Mission. Primary role will be to provide crisis care clinical services for appropriate patients as assigned, as well as visit home and facility team patients as needed. This role identifies and coordinates patient and family needs, providing preventative, palliative, and supportive care to terminally ill patients and their families.

This role will collaborate with the Team RNs and other disciplines to ensure the highest quality of care is maintained.

EDUCATION AND

QUALIFICATIONS:

• Currently licensed in the State of Florida as Licensed Practical Nurse (LPN).
• Must maintain current American Heart Association approved CPR certification.
• Minimum of one (1) year nursing experience.
• Hospice, Oncology, or Home Health experience is preferred.
• Minimum of One (1) year acute care nursing experience with training in Symptom Management.
• Most possess and maintain a current Florida driver’s license.
• Most possess and maintain automobile insurance coverage.
