- Provide direct patient care in accordance with the state Nurse Practice Act, including end-of-life care
- Implement and support individualized plans of care under RN supervision
- Deliver effective pain and symptom management, evaluating patient response to treatment
- Educate patients and caregivers on disease progression, comfort measures, and care needs
- Maintain a safe, clean, and supportive care environment
- Document care accurately and timely in the EMR
- Partner with physicians and interdisciplinary team members (RN, HHA, Social Work, Chaplain, etc.)
- Participate in care coordination and team discussions to ensure holistic patient support
- Build trusted, compassionate relationships with patients and families
- Uphold the highest standards of clinical practice and professionalism
- Participate in quality improvement and team initiatives
- Engage in ongoing professional development
- Active LPN/LVN license in the state of practice
- Current CPR certification
- 1–3+ years of nursing experience preferred (hospice, home health, or med-surg a plus)
- Strong communication skills and emotional intelligence
- Ability to provide compassionate, patient-centered end-of-life care
- Valid driver’s license, reliable transportation, and auto insurance
