Social Worker, Mental Health

Ark Healthcare and Rehabilitation at Branford Hills, a premier 190-bed Skilled Nursing Facility seeks a Full-Time Social Worker to join our team!

As a member of the Social Work Department you will play an integral role in enhancing the well-being of our residents and help them meet their basic and complex needs.

Responsibilities:

• Develop social, psychosocial history, formulate care plan which addresses problems, needs, and concerns.
• Participate in Interdisciplinary Meetings
• Maintain compliance with MDS assessments, care plan review and revision.
• Maintain compliance with federal and state documentation requirements .
• Inform, educate, and advocate on behalf of patients and their resident rights.
• Coordinate discharge planning, and assess patients for the potential of discharging.
• Provide counseling to residents
• Maintain compliance with required state assessment schedule, e.g. Assessment Pro Level I, IIs, Level of Cares and communicate changes or pertinent information to IDT.
• Serve as point of contact with hospice as well communal agencies and care providers for post-discharge environments.
• State/Federal Survey experience
• Participate in Quality Assurance and Performance Improvement Initiatives (QAPI) Program.
• Communicate with Probate Court in matters related to Conservatorships

Other Duties as assigned.

Skills:

Must possess a BSW, MSW, or have qualifications to serve as a designee detailed in CT DPH §19-13-D8t Public Health Code.

Must possess working knowledge of Healthcare /SNF/LTC Social Work practices

Must possess strong work ethic, communication skills, and writing skills.
