** Join our compassionate team as an
** ACT Therapist**, where you will deliver community-based, recovery-oriented mental health services to individuals with serious and persistent mental illness. In this role, you will work as part of a multidisciplinary Assertive Community Treatment (ACT) team, providing intensive, individualized support to clients in their homes, workplaces, and communities.

As an ACT Therapist, you will assess, diagnose, and treat clients utilizing evidence-based interventions while promoting independence, stability, and improved quality of life. This position requires flexibility, strong collaboration skills, and a commitment to meeting clients where they are—both clinically and physically. You will play a vital role in reducing hospitalizations, supporting crisis intervention, and helping clients achieve their personal recovery goals.

* Key Responsibilities:

*** Provide
** community-based individual, family, and group therapy
** in homes and community settings
* Conduct intake screenings and comprehensive assessments within designated time frames
* Diagnose mental health conditions based on clinical assessments
* Develop and implement
** individualized treatment plans
** in alignment with ACT model standards
* Deliver
** crisis intervention and on-call support
** as part of the ACT team rotation
* Participate in
** daily team meetings and clinical staffing**, contributing to shared caseload decisions
* Document all services in compliance with regulatory and ACT program standards
* Assist clients with
** life skills development**, including housing stability, employment, and social integration
* Coordinate care with psychiatrists, nurses, case managers, and other team members
* Support transitions to higher or lower levels of care as appropriate
* Maintain strong, professional relationships with clients, families, referral sources, and community partners
* Provide clinical consultation and support to team members when needed
* Other duties as assigned by leadership
** Education, Experience, and/or Credential

Qualifications:

*** Master’s degree in Social Work, Counseling, Psychology, or a related field required
* Active licensure (LPC, LCSW, or LMFT) in the state of practice required
* Willingness to obtain licensure in additional states as needed
* Experience working with individuals with serious mental illness preferred
* Comfortable providing
** telehealth and in-person community-based services

* Additional Qualifications:

*** Successful completion of background checks (criminal, driving, abuse/neglect, fingerprinting)
* Valid driver’s license, reliable transportation, and current auto insurance (required for community work)
* First Aid and CPR certification within three (3) months of hire
* Behavioral Management training within thirty (30) days of hire
* Knowledge of ACT model principles, community resources, and mental health regulations preferred

* Physical Requirements:
