Full Time Home Health Physical Therapist
Full Time Home Health Physical Therapist based near Branson, Missouri. Provides individualized therapy in patients' homes, improving strength, balance, and independence. Offers a stable, permanent role with flexible autonomy and no weekend work.
Key Responsibilities:- Perform comprehensive patient evaluations and develop individualized treatment plans
- Conduct start-of-care assessments and complete OASIS documentation accurately
- Provide routine home health therapy visits focused on improving strength, balance, mobility, and independence
- Complete patient recertifications and reassessments as required
- Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams
- Monitor patient progress and modify treatment plans to maximize outcomes
- Maintain timely and accurate clinical documentation using Matrix EMR
- Deliver compassionate, patient-centered care in a home-based setting
- Doctorate or Master's degree in Physical Therapy from an accredited program
- Active Missouri Physical Therapist license
- Strong communication, organization, and time-management skills
- Ability to work independently and manage a daily patient schedule
Skills:
- CPR/BLS certification (preferred)
- Experience with OASIS documentation (preferred)
- Familiarity with Matrix EMR (preferred)
