Perm Primary Care Nurse Practitioner

Position Type:
Permanent, Full-Time (1.0 FTE)

You will work in a collaborative outpatient environment focused on comprehensive health promotion, disease prevention, and long-term wellness.

• Primary Care Delivery:
  Independently perform medical acts in accordance with APRN licensure standards for individuals, families, and groups.
• Collaborative Care:
  Work alongside a multidisciplinary team including physicians, specialists (Cardiology, Orthopedics, OB/GYN, etc.), and support staff to optimize patient outcomes.

  Identify specialized learning needs and provide clinical education to support informed healthcare decision-making for patients and families.
• Clinical Leadership:
  Provide consultation within the care team and support daily practice operations in a cost-effective, safe, and efficient manner.
• Professional Growth:
  Participate in ongoing professional development and establish personal goals for clinical excellence.

• Hours:
  
  Monday–Friday, 08:00 – 17:00 (40 hours per week).
• Weekend Requirements:
  None.
• On-Call: 1:11 rotation (Light community-based call).
• Environment:
  Outpatient only; multidisciplinary medical group setting.

• Education:
  
  Master’s Degree from an accredited Nurse Practitioner program [REQUIRED].
• Licensure:
  Current Vermont APRN License (or eligible to obtain) [REQUIRED].
• Certifications:
  • National Certification as Adult, Family, or Pediatric NP [HIGHLY PREFERRED].
  • Healthcare Provider CPR and active DEA License [REQUIRED].
• Experience:
  
  Two years of nursing experience prior to obtaining an advanced degree [HIGHLY PREFERRED].
• Documentation (Must be provided at time of submission):
  • Most recent NPDB Self-Query Report (Active within 30 days).
  • Clean Malpractice History.
