Vermont APRN license required - Experience:
Minimum 1 year of outpatient primary care experience
- Salary Range: $120,000 - $140,000 annually, based on experience and qualifications

- Independently perform medical acts aligned with APRN licensure standards in outpatient primary care settings

- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes
- Educate patients and families to support informed decision-making and health management

- Lead health initiatives and offer consultation within the care team
Required:
- Vermont APRN license
- Minimum 1 year of outpatient primary care experience
- Strong communication and teamwork skills
- Experience working in a multidisciplinary outpatient team
- Passion for patient education and holistic care
