The pay for this position is $44,850-$58,909 annually.
$500 sign-on bonus

The Community Case Manager works directly with individuals receiving services to build functional skills, address barriers to care and social and mental wellbeing, and support follow-through on treatment plans and referrals. Our ideal candidate will have strong collaboration, problem-solving, and interpersonal skills.

- An Associate's degree and two years experience and/or a bachelor's degree
- Knowledge on how to provide holistic and therapeutic case management services.
- Experience working in the mental health field.
- Computer and documentation skills.
- Ability to solve problems and deal with complex situations and stay calm.
