Advanced Care Partners is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for medically fragile children and adults in home and school settings. This role offers the opportunity to deliver high-quality, individualized care in a more focused and meaningful environment.
Base Pay: $35/hour Opportunity to earn up to $43/hour with applicable differentials:
- Trach: +$2/hour
- Vent: +$3/hour
- Nights (7pm–7am): +$2/hour
- Weekends: +$3/hour
- Holidays: +$3/hour

Many of our nurses join through referrals and remain with ACP due to the quality of care delivery and team support.
Key Responsibilities
- Deliver skilled nursing care, including G-tubes, tracheostomies, ventilators, and central lines
- Monitor patient condition and respond appropriately to changes
- Educate patients and families on care plans and procedures
- Collaborate with interdisciplinary team members to ensure optimal outcomes
Active LPN license in Georgia or Nurse Licensure Compact Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or an equivalent accredited organization is required. A minimum of one year of LPN experience required. Valid PPD skin test
Physical Requirements of the Role
- Ability to stand or sit for extended periods (up to 12 hours)
- Ability to bend, twist, kneel, and assist with patient mobility
- Ability to lift, push, or pull up to 50 pounds or more
- Manual dexterity for clinical tasks
