Licensed Vocational Nurse (LVN) | Bremerton Trails Post Acute | $40–$50/hr

Bremerton Trails Post Acute is hiring an LVN to provide hands‑on care for chronically ill and technologically dependent residents. Administering medications, managing complex treatments, and working closely with our RN and interdisciplinary team to deliver individualized, resident‑centered care.

What You'll Do
  • Deliver total care for chronically ill and technologically dependent residents, factoring in physical, psycho‑social, and spiritual needs
  • Complete initial and ongoing resident assessments, distinguishing normal from abnormal clinical findings and intervening per clinical standards and physician orders
  • Collaborate with residents, families, and the interdisciplinary team to build individualized care plans
  • Administer medications, including pain management, and provide IV care within your scope if IV certified
  • Perform treatments per physician direction — wound packing, irrigation, indwelling catheter care, and more
  • Manage care for technologically dependent residents, including ventilator dependency, ostomies, enteral feeding tubes, tracheostomy care, and suctioning
  • Monitor and report condition changes promptly to the RN, physician, family, and care team
  • Provide resident and family education tailored to individual needs, and document its effectiveness
  • Recognize and report signs of abuse or neglect per facility policy and regulatory guidelines
  • Maintain thorough, accurate documentation in accordance with facility and regulatory standards

What You'll Need
  • Active LVN license in good standing
  • Experience with technologically dependent residents (ventilators, ostomies, feeding tubes) preferred
  • Strong clinical assessment skills and sound nursing judgment
  • Clear, effective communication with residents, families, physicians, and the care team
  • Ability to stand, walk, bend, and lift up to 25 lbs regularly (up to 100 lbs with assistance)
