What You’ll Do (AKA: Your Superpowers) Own the Assessment Process:- Complete timely and accurate Minimum Data Set (MDS) assessments in compliance with federal, state, and community guidelines.
- Ensure thorough, collaborative interdisciplinary care planning for each resident.
- Maintain detailed documentation that reflects resident care, progress, and reimbursement accuracy.
- Monitor clinical documentation for accuracy and support staff in completing assessments and charting.
- Review and interpret clinical data to identify changes in resident status and adjust care plans accordingly.
- Coordinate with reimbursement and billing teams to ensure appropriate coding and timely submissions.
- Serve as the go-to expert on MDS processes and educate team members on documentation best practices.
- Partner with nursing, therapy, and administrative staff to support quality outcomes and survey readiness.
- Participate in QA meetings, audits, and other quality improvement initiatives.
- Current RN license in the state of employment required.
- RAC-CT certification preferred or willingness to obtain within designated timeframe.
Skills:
- Prior experience in MDS coordination within a skilled nursing setting required.
- Strong knowledge of RAI process, PDPM, Medicare guidelines, and care planning.
- Excellent critical thinking, time management, and communication skills.
- Ability to sit, walk, bend, and use computer systems for extended periods.
- Must be able to lift up to 25 pounds occasionally.

Salary Range:LPN $57,302.00-$78,367.99
RN $63,400.00-$86,707.81
