Murfreesboro, TN. Coverage area:
Rutherford and Williamson counties.

Schedule:

Monday through Friday, 9:00 am to 6:00 pm.

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care.
• Explain hospice services, discuss goals of care with the patient and families, answer questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptom information.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• One year of hospice nursing experience required, 3+ years preferred, or similar setting such as long‑term care, intensive care, or emergency room considered in place of hospice experience.
• Must be a licensed driver with a vehicle that is insured, in good working order, and meets state/or organization requirements.
