Licensed Practical Nurse Outpatient PACE Program

This position is based in an outpatient clinic and operates Monday-Friday, closed on weekends.

The Licensed Practical Nurse (LPN) within the PACE program plays a vital role supporting participants through comprehensive medication oversight, team coordination, and hands‑on care. Guided by the PACE model of care, the LPN ensures medications are administered and documented accurately, assesses changes in condition, and collaborates with providers, pharmacy, caregivers, and the interdisciplinary team to promote optimal participant outcomes.

• Provide direct patient care in an outpatient setting and evaluate outcomes.
• Evaluate and record patient vitals.
• Assist the provider with therapeutic and diagnostic procedures.
• Administer oral/ injectable medications as ordered and monitor patient response.
• Delegate to licensed and unlicensed staff according to scope of practice.
• Deliver quality customer service to patients and family members.
• Triage participants and determine need for physician review based on urgency.
• Coordinate care with specialist providers and nursing facilities.
• Collaborate with the care team to implement goals and interventions.
• Create and update care plans based on participant needs.
• Document and report data, patient response, and progress regularly.
• Individualize patient teaching and care plans.
• Advocate for patients and families throughout the continuum of care.

• Licensed Practical or Vocational Nurse (LPN/LVN).
• Associate Degree or equivalent.
• Basic Cardiac Life Support certification to be obtained within 30 days of employment start date.
• Certification to be obtained within 2 years of employment start date.
• Travel not required.
