Position: LICENSED PRACTICAL NURSE

Care Partners is seeking a compassionate Licensed Practical Nurse to join our Inpatient Rehab team within our Healthcare & Medical Services department. In this role, you will provide direct patient care, administer medications, monitor patient progress, and support rehabilitation plans under the supervision of RNs and physicians. You'll collaborate closely with an interdisciplinary team to deliver patient-centered care, educate patients and families, and document accurately in medical records.

Responsibilities

  • Provide direct bedside nursing care to adult and geriatric patients in an inpatient rehab hospital setting within LPN scope of practice.
  • Perform focused nursing assessments, monitor vital signs, and observe and report changes in patient condition to the RN and provider team.
  • Administer medications and treatments safely and accurately, following hospital policy and state regulations.
  • Assist patients with activities of daily living, mobility, and rehabilitation-focused tasks, reinforcing therapy goals set by PT/OT/SLP.
  • Implement and support individualized care plans aimed at improving functional status and preparing patients for safe discharge.
  • Document all care, assessments, and patient responses accurately and promptly in the electronic health record.
  • Collaborate with physicians, RNs, therapists, case managers, social workers, and other staff during rounds, huddles, and care conferences.
  • Educate patients and families on medication regimens, disease processes, rehab goals, and post-discharge care instructions.
  • Follow infection prevention, fall prevention, and skin-integrity protocols to promote a safe environment and prevent complications.
  • Participate in quality improvement and patient-safety initiatives and comply with all hospital and regulatory standards.

  • Work flexible shifts, including some evenings, nights, weekends, and holidays, to support 24/7 inpatient operations.
