Social Services Case Aide
Latham Centers is seeking a Social Services Case Aide to support children and families through coordination of social services, case management, and clinical support. In this role, you will work closely with clinicians, families, school districts, and state agencies to ensure student needs are met and documentation is maintained accurately and timely.
Location: Brewster, MA
Schedule: Monday-Friday 8:00am-4:00pm
Compensation: $20.75 - $22.30 per hour (based on experience)
Key Responsibilities- Schedule all clinical meetings for assigned caseload, including coordination with clinicians, school districts, state agencies, and families
- Complete ordering for students, including vocational stipends and additional approved needs
- Monitor student accounts and follow up with families regarding balances and financial needs as appropriate
- Manage all aspects of visit scheduling, including calendar input, travel confirmation, transportation requests, and staffing coordination
- Attend student meetings including MDTs, IEPs, and treatment reviews
- Complete discharge follow-up for assigned caseload and document accordingly
- Assist with transportation of students to and from home visits in collaboration with Scheduling
- Enter incident reports for assigned caseload in the DCF portal
- Ensure all MDT and Quarterly Meeting documentation is submitted and correctly saved and labeled in student files
- Address family needs and concerns as they arise
- Complete annual non-medical consents for assigned caseload
- Schedule and attend BSP meetings in coordination with clinicians and behavior technicians
- Attend educational and residential component meetings regularly
- Supervise visits as directed by DCF
- Maintain and update student IDFs and communicate changes to the administrative team for fact sheets
- Maintain and update student phone call lists for assigned caseload
- Download approved music and content onto electronic devices as funded by parents or guardians
- Participate in a rotating schedule of note‑taking at component meetings
- High School Diploma or GED
- Valid Driver’s License
- Ability to safely and compassionately manage children in crisis situations
