BROAD RESPONSIBILITIES
The Transition Coordinator is responsible for providing comprehensive casework services for transitioning RTC youth and their families/guardians. They will be responsible for providing community-based linkages and supports to treatment and other services beginning at the intake phase and through the transition back to the community. Services will be provided to the RTC youth and their families/guardians in their home communities and will be coordinated with all necessary community providers of services.
Education/Experience:
Bachelor’s Degree in Human Services and 2 years’ experience providing direct services to or linking children with serious emotional disturbance to services.
Licensure/Certification :Valid Driver’s License
Schedule:Monday through Friday hybrid remote schedule. Weekend hours required at varying times throughout the year to facilitate family and/or parent activities. Hours outside of normal schedule required for parent meetings, conferences and other department and/or agency related activities as needed.
Starting Salary:$46,020 per year
Note this is a temporary role and is not benefit eligible. It is anticipated to begin in April and last several months.
