The Transition Coordinator is responsible for providing comprehensive casework services for transitioning RTC youth and their families/guardians. Theywill be responsible for providing community-based linkages and supports to treatment and other services beginning at the intake phase and through the transition back to the community. Services will be provided to the RTCyouth and their families/guardians in their home communities and will be coordinated with all necessary community providers of services.
Education/Experience:
- Bachelor’s Degree in Human Services and 2 years’ experience providing direct services to or linking children with serious emotional disturbance to services.
- Valid Driver’s License
This isa full-time and benefit eligible role with a salary of $46,020 per year
