** SUMMARY (BASIC PURPOSE OF THE JOB)
** Promotes a safe and cost-effective medication management system for patients at Capital Health (CH) within his/her assigned practice area. Provides education, information and publications on pharmaceutical care and drug therapy. Develops and supports quality initiatives through data management and analysis. Works in collaboration with Pharmacy staff to effectively operate the department through policy/protocol development and liaison with medical, nursing, and ancillary communities.
Assumes pharmacist responsibilities as necessary to assure smooth operations of the Pharmacy Department.## ##
** MINIMUM REQUIREMENTS
**** Education**:
Graduate of an ACPE-accredited school of pharmacy. Doctor of Pharmacy (Pharm
D). Board of Pharmacy Specialties (BPS) certification preferred.
** Experience**: PGY1 Pharmacy Residency or three years equivalent experience required. PGY 2 Emergency Medicine or Critical Care preferred.
** Other Credentials:
** AHA ACLS-Adv Cardiac Life Supp, AHA BLS
- Healthcare Provider, AHA PALS, Pharmacist
- Registered
** 2 hours of Stroke related Continuing Education
** contact hours annually
** Knowledge and Skills**:
Possesses excellent organizational, interpersonal, conflict resolution, verbal, and written communication skills. Ability to effectively manage multiple projects and responsibilities simultaneously. Ability to respond quickly in a fast paced environment.
** Special Training** :
Intermediate to advanced computer skills. ASHP-accredited pharmacy residency training preferred.

* Interprets, evaluates, and accurately processes drug orders for patients receiving care in assigned areas. Efficiently and effectively utilizes pharmacy operational computer systems and integrated hospital systems. Follows policies and procedures to maintain accurate patient medication profile. Initiates appropriate medication therapy based on age, disease, and drug considerations for neonatal, pediatric, adolescent, adult, and geriatric patients. Actively intervenes to assure therapy is individualized based on medication factors and the patient’s physical and psychological needs.
* Identifies and resolves problems with medication orders or therapeutic regimens. Clarifies issues with prescribers and suggests order adjustments or alternative medication therapies when needed to prevent adverse events or adheres to institutional policies.
