- • Establish, implement, and maintain Care Management process components focused on satisfactory patient outcomes
- • Ensure services comply with regulatory agency requirements and Conditions of Participation
- • Implement regulatory standards related to Care Management and continuum of care/discharge planning
- • Provide direct clinical and administrative direction to Inpatient Care Management staff at assigned regional hospitals
- • Provide educational training for Care Management personnel, clinical staff, and ancillary departments
- • Oversee compliance and quality audits of direct care manager reports, including workflow adherence, software use, documentation, and training needs
- • Represent Care Management at designated service line Performance Improvement meetings, community organizations, and external services
- • Monitor data, including variance analysis and aggregate trend analysis, and recommend practice changes
- • Report to and participate in the Network Utilization Management Committee for assigned regional hospitals
- BSN or MSW required
- If RN, must maintain RN licensure in Pennsylvania
- MSN preferred if RN
- Ability to drive a car
- Ability to meet physical demands including sitting, standing, walking, climbing stairs, and frequent fingering/handling
- Ability to lift and carry objects up to 10 pounds frequently
- Ability to occasionally lift, carry, push, and/or pull objects up to 75 pounds
- Ability to hear normal conversations and high and low frequencies
- Ability to see with general and peripheral vision
- Ability to use touch for telephone and computer keyboard tasks
Demonstrates expertise in Care Management processes, regulatory compliance, and quality audits while providing clinical and administrative direction to staff. Capable of analyzing data trends and implementing practice changes to enhance patient outcomes.
