Complete Care – Essex County, NJ

Complete Care is seeking an experienced and compassionate External Case Manager to support our skilled nursing facilities throughout Essex County, New Jersey
. This role is responsible for building strong relationships with hospitals, healthcare providers, and community partners while helping coordinate appropriate transitions of care and supporting census growth.

• Develop and maintain strong relationships with hospitals, physicians, case managers, social workers, and other referral sources.
• Coordinate and support patient admissions and transitions from hospitals and other healthcare settings.
• Serve as a key liaison between Complete Care facilities and external healthcare partners.
• Identify opportunities to strengthen referral relationships and increase admissions.
• Collaborate with facility leadership, clinical teams, and business development to support census goals.
• Conduct regular outreach and follow-up with referral sources.
• Represent Complete Care professionally within the healthcare community.
• Track referral activity and maintain accurate records of relationships and opportunities.

• Experience in healthcare, case management, hospital relations, admissions, business development, or a related field.
• Knowledge of skilled nursing, post-acute care, or hospital discharge processes strongly preferred.
• Strong communication, relationship-building, and organizational skills.
• Ability to work independently while collaborating effectively with facility and regional leadership.
• Must be comfortable traveling throughout Essex County and visiting referral partners.
