TAG Med Staffing is seeking a Social Worker / Geriatric Care Manager for a full-time role in New Jersey. You will manage a caseload of community-dwelling seniors, with most cases in Bergen County, NJ, and occasional cases in South Jersey and New York.
Hybrid schedule includes 1 day in office for team meetings and remote field visits. Ideal candidates have LMSW or LCSW, strong MSW background, and experience with Medicare, Medicaid, entitlements, and community services.
