As a Physical Therapist, you will evaluate residents, develop individualized treatment plans, and implement therapeutic interventions designed to restore function, improve mobility, enhance independence, and maximize quality of life. We are looking for professionals who are passionate about making a meaningful impact while delivering patient-centered care in a collaborative environment.

Key Responsibilities
  • Evaluate residents and clients to determine functional limitations, therapy needs, and appropriate treatment goals.
  • Develop, implement, and modify individualized treatment plans based on assessments and ongoing progress.
  • Conduct physical therapy treatments focused on mobility, gait training, ambulation, strength, balance, coordination, endurance, posture control, and functional independence.
  • Utilize therapeutic exercises, modalities, and evidence-based interventions to improve physical performance and overall well-being.
  • Design, fit, and train clients in the use of adaptive equipment, orthotics, and other assistive devices as appropriate.
  • Educate residents, families, and caregivers regarding treatment plans, safety recommendations, and home exercise programs.
  • Collaborate with physicians, nurses, rehabilitation staff, and interdisciplinary team members to ensure comprehensive care.
  • Provide supervision to Physical Therapist Assistants, including case conferencing, co-signing documentation, and supervisory visits as required.
  • Attend care plan meetings, interdisciplinary meetings, and other required departmental meetings.
  • Complete all clinical documentation accurately and in a timely manner.
  • Conduct training and education for staff regarding therapy techniques and rehabilitation objectives.
  • Maintain therapy equipment and report any repair or maintenance needs.
  • Ensure residents and families receive services in a caring, compassionate, and respectful manner.
  • Support the mission, vision, and values of FellowshipLIFE while promoting excellence in resident care.
Qualifications
  • Graduate of an accredited Physical Therapy program.
  • Current New Jersey Physical Therapist License required.
  • Valid New Jersey Driver's License required.
  • Minimum one year of experience preferred; new graduates are encouraged to apply.
  • Strong clinical assessment and treatment planning skills.
  • Excellent interpersonal, verbal, and written communication skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Commitment to providing exceptional resident-centered care.
