Job DescriptionMetro is looking for a motivated and passionate FULL TIME Physical Therapist to provide in-home care. If you are a Physical Therapist ready to take the next step forward in your career, we want to hear from you. Responsibilities- Accept and treat patients within your designated geographic area.
- Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
- Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
- Plan, prepare and implement customized therapy programs to restore function and promote independence.
- Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
- Provide timely and accurate documentation for each visit.
- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
- Deliver care in compliance with agency policies and professional practice standards.
- Physical Therapist Program Graduate.
- Licensed Physical Therapist in the state of New Jersey or eligible.
- Home health experience is preferred but not required; mentorship will be provided for documentation and compliance.
- Ability to travel to patients’ homes within the assigned geographic territory.

The anticipated base salary range for this position is $90,000.00 – $. Salary is based on various factors, including relevant experience, knowledge, skills, other job‑related qualifications, and geography. Additionally, this position is eligible for incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
